Notice! Free Shuttle Service for Bourbon High School Graduation. Buses will run approximately every 10 minutes from the Elementary/Middle School Parking lot located at 357 Jost Street beginning 6:30 for the program beginning at 8:00 p.m. Shuttle will also be available at the end of each program to transport riders back to their cars. |

Click below for School Supply List for grades PK-4 (Elementary) & 5-8 (Middle School) Add Immunizations to School Supply Lists - Required for Kindergarten through 12th Grade
Immunizations are an important part of a child’s health, and Missouri law requires children enrolled in kindergarten through 12th grade to be immunized against certain vaccine-preventable diseases.
School-aged children must be immunized against diphtheria, tetanus, pertussis, polio, measles, rubella, mumps, hepatitis B, and varicella.
Individuals attending kindergarten through 12th grade are required to provide proof of up-to-date immunizations at registration. State regulation specifies that an individual cannot attend until all required immunizations are current.
Public, private, parochial and parish schools are required to know the immunization status of the children enrolled in the facility. This information not only protects children but also the staff from harmful and potentially deadly vaccine-preventable diseases.
It is the responsibility of the school to maintain the immunization histories of all the children enrolled and to report that information to the Missouri Department of Health and Senior Services by October 15th of each year.
The Missouri Department of Health and Senior Services has developed a Back to School campaign for the 2012-2013 school year, which includes a number of resources to help remind parents/guardians to get a child’s immunizations prior to school registration.
These resources include a bookmark, reminder postcards (both hardcopy and ecards), fact sheets and information on the Vaccines for Children program. They are available at no cost and can be ordered at health.mo.gov/immunizations or by calling 573.751.6124. |

SAVE TIME - FILL OUT STUDENT PAPERWORK IN THE COMFORT OF YOUR OWN HOME
We would like to offer our parents the opportunity to fill out 2012/2013 student paperwork for new student enrollment.
Families should fill out only one Household Information form for all students residing in their home, as well as filling out one Student Form for each child enrolling new to the district (Kindergarteners and transfers). This gives you the opportunity to fill out the information in the privacy of your home and take time to fill them out with important phone numbers, etc.
You will likely be asked to complete additional forms at open house that will include a Free/Reduced Lunch Application. Please complete the Free/Reduced Lunch Application even if you don't think you qualify.

CHECK THIS OUT! The Department of Elementary and Secondary Education is pleased to announce the release of the "Top 10 by 20--Missouri Proud" video. This six-minute video introduces the Top 10 by 20 initiative and provides an overview of the importance of Missouri becoming one of the Top 10 states in academic performance by the year 2020.

BATTERY RECYCLING The PTO is making it our mission in FEBRUARY to start to help get rid of all TYPES of batteries. We will be placing in the Elementary, Middle, and High School's a BOX for batteries. Please help our future planet by placing batteries in the BOXES. Neo-Boards are going to have old batteries that are going to be need to be disposed of - what a great place to start - right here at school! We will also take any shape or size battery from home, for those that wish to bring them in to be recycled. Any questions regarding this program can be emailed to PTO@mail.warhawks.k12.mo.us. PLEASE RECYCLE ALL SHAPES AND SIZES BATTERIES FROM CLASSROOMS, OFFICE AND HOME. BATTERY Boxes can be found in Teacher workrooms and at the entrance by parent pick-up near elementary office. The planet, our children and grandchildren thank you. Remember that a little now goes a long way in the future!
